Forms Needed for Participation

Here are the forms that need filling out to participate on the Alpine Ski Racing Team: 

Please Print them out, fill all information, and Sign where necessary.  Mr. Hanbury can be the witness for the Mount Southington Waiver Form and the Rules Form.  Please Return these forms ASAP to Rm 248 or email scanned copies to rhanbury@berlinschools.org no later than Dec. 12, 2012.
